We have developed these REWIRE preparation packs for you to help us all be well prepared for our assessment meeting.  Please read and complete the forms and return them to us - it is essential this is done before our appointment.  There is a pack of forms for clients who are adults & a pack of forms for parents/caregivers to complete where the client is a child. There is also a special Brain Bloom Room for kids resource that we have created to help kids (young & old) learn more about what we do along with information about our approach.  We hope you enjoy it.
